• eighteen copies (signed original plus seventeen photocopies) of the completed Research Ethics Application form including all attachments.
  
  
• eighteen copies (original plus seventeen photocopies) of english consent forms and/or information sheets and advertisements, if applicable.
  
  
• eight copies of Pharmaceutical Company Protocols.
  
  
• eight copies of Questionnaires, Case Report Forms, etc.
  
  
• two copies of Investigator's Drug Brochure, if applicable.
  
  
• eighteen copies of the detailed budget.
